Friday Freebie: Health Record Chart

The first question I ask my clients when they meet with me online, in person or by phone for their appointment is “What are your daily habits?”

The average client cannot answer this question.

Can you answer that question with accurate detail?

Do you remember exactly what you ate for the past week, how many hours you slept, what herbs or remedies you consumed exactly and how much exercise you got?

Most people will not be able to remember these things in detail but they are key to unraveling the reasons behind why you are feeling ill and essential to finding your path to better health and well-being.

Keeping a chart like this (download our free chart below) cuts down on consulting time with your health care professional and can provide them with much more accurate data than they can usually gain in an appointment. Having accurate data means:

  1. You need fewer appointments to solve your problem
  2. You pay for fewer consulting or appointments
  3. You save time
  4. You avoid potentially harmful contraindications in treatment
  5. You receive more personalized treatment options
  6. You can often solve problems without assistance

In fact, many clients have reported that once they started keeping a daily chart they were able to solve many of these “mysteries” on their own – without outside help.

Clients have reported being able to accurately pinpoint their allergies, food and environmental sensitivities, their ideal diet, ideal sleep times and hours, and even answer questions for themselves like: “Why does my stomach hurt?” or “Why do I often get headaches?”

With all these benefits you might assume that keeping daily data will take a large commitment on your part. You may have even tried out one of those health-bands or apps that help you track your daily data. But many people report that their bands and apps are too “expensive”,“high maintenance”, “too confusing” or “provide too much data”.

Keeping a simple chart takes only 5-10 minutes a day and is actually more effective and useful.

How? The secret is:

  1. Know what to put on the chart
  2. Keep chart descriptions detailed so your daily record keeping is reduced
  3. Keep printable charts ready or keep one on your smartphone or tablet
  4. Keep the chart in a location that is easy to access
  5. Keep filling in the chart even if you miss a day
  6. Keeping the data simple allows it to stay manageable and easy to interpret
  7. If additional data is needed after initial analysis you can personalize your chart
  8. Does not require batteries, expensive wristbands, a smart phone or an app
