This listing’s price is per student and per certificate & transcript set. If you need documents for more than one student and/or grade, you will need to order more than one set and fill out more than one “Document Request Form” (below).

We only start on your order once you have both (1) made your payment and (2) filled out and submitted the form below. We do not accept document requests by email. Please indicate any additional information in the “Notes” section of the form. We will reach out to you with any further questions we have and with drafts for you to approve before finalizing!

Including MONTH, DAY, and YEAR. I will read dates in this order — MM/DD/YYYY — we are based in the US! You may spell out the date instead or use the “notes & special requests” field at the bottom of this form to provide more details as applicable. You will always receive a draft of your documents to approve before your order is finalized.

e.g. listing different grades (other than As), adding and removing classes, changing the format of the documents for government/school requirements

On “Notes & Special Requests”: You may request no more than two types of changes to the standard document templates. For example, if you want to change the grades your student earned in their classes to something besides “A”s (mastery), this is one type of change, and you do not need to “use up” your two changes for each class you want a different grade for. “Types” of changes include:

  • changing the grades your student earned in their classes to something besides “A”s (mastery)
  • adding or removing written curriculum classes
  • adding or removing living lessons curriculum classes
  • listing significant, external independent study e.g. private tutoring or training, community classes or team sports, internship or field placement
  • adding or removing from student information (transcript, upper righthand corner)
  • minor document formatting adjustments in alignment with school/government requirements

If you exceed your allotted changes or if any of these changes to the standard document templates require more than two sets of drafts to be issued before finalizing, you may need an ADVANCED Digital Diploma & Transcript set.
